If you didn't finish the medical examination requirements in school, to avoid delaying in receiving the report, please go to China Medical University Hospital and complete the Health checkup.

Registration：China Medical University Hospital / Preventive Medicine Center 8F
Time：Mon.~Fri.　8:00 AM - 11:30 AM    /    1:00 PM - 4:30 PM
　　　     Sat.　    　   8:00 AM - 11:30 AM   /     1:00 PM - 3:30 PM

Please take the 「Medical Examination Requirements Form」, 「Student ID Card」, and 「Resident Permit Card」to check-in
